Description
The Population Health Strategy Lead is responsible for improving the quality of care and outcomes while managing costs for a defined group of people most impacted by health disparities and health inequity. The Population Health Strategy Lead works on problems of diverse scope and complexity ranging from moderate to substantial, in collaboration with multiple departments across the enterprise.
Responsibilities
The Population Health Strategy Lead identifies health needs such as chronic diseases or disabilities or the health needs of the underserved. This position will initially focus efforts in West Louisville, to improve health outcomes and enable everyone to have a fair and just opportunity to be as healthy as possible. Advises executives to develop functional strategies (often segment specific) on matters of significance. Exercises independent judgment and decision making on complex issues regarding job duties and related tasks, and works under minimal supervision, Uses independent judgment requiring analysis of variable factors and determining the best course of action.
The Population Health Strategy Lead will work cross-functionally with Humana clinical teams (e.g., Medical, Nursing, Pharmacy, Social Work) and business units (e.g., Medicaid, DH&A) and with external strategic partners to establish and support strategic academic partnerships.
Responsibilities:
Partner closely with a team to execute strategic relationships.
Work internally to develop and manage governance processes and to co-create and execute strategies for clinical partnerships
Identify strategic priorities and work to implement those strategies with the appropriate internal and external partners across multiple business segments, academic partners, and external partners
Define and work with appropriate professionals to conduct research projects that result in quantifiable health outcomes improvement and publishable studies
Develop key metrics and report on those metrics to senior executives internally and with strategic partners externally
Collaborate with data teams to establish appropriate protocols for leveraging Humana's data assets with strategic partners for research
Build positive relationships to achieve strong, sustainable results for Humana, our partners, and the people we serve
Work with internal and external partners to establish sustainability in practices that lead to improved whole person health
Required Qualifications
Bachelor's Degree
Preferred Qualifications
10 years in a health and well-being environment or equivalent experience
Demonstrated ability to work with senior executives across multiple functions
Exceptional communication skills and strong presence, with the ability to influence without authority
Sound decision making, working in an independent and unsupervised environment
Proven collaboration skills
Understanding of integrated care delivery and how to bring diverse aspects of the care system into a holistic model to make it easy for people to achieve their best health
Self-starter who motivates others to deliver their best
Agility and willingness to adapt to varied work environments.
Strong experience building relationships and collaborating
Additional Information
The position will be based remotely but will have some travel expectation (25-30%) when conditions permit.
