Provides moderately complex (journey-level) case management work in a fast-paced environment to meet the needs of individuals with intellectual and developmental disabilities; Work involves developing and maintaining long-term contact with clients, client families, and service providers for medical, behavioral, social, educational, and related service needs; May train others; Works under general supervision, with moderate latitude for the use of initiative and independent judgment. Orients new and existing service coordinators with specific service coordination job functions and activities, and existing staff on an ongoing basis; Ensures that all service coordinators receive mentorship training within 30 days from the first day they report to the unit and at least quarterly on an ongoing basis; Interviews and screens clients or authorized representatives to gather information to assess service needs, complete intake/enrollment activities, and determine diagnostic and financial eligibility; Develops, implements, and monitors service and treatment plans to meet clients’ needs consistent with the Person-Directed Planning philosophy; Coordinates service provider and program activities; Links and coordinates services available through community resources; Provides ongoing targeted case management and serves as a liaison between clients, client families, and service providers; Provides crisis prevention and management; Identifies clients’ needs, problem areas and service gaps, and areas that are barriers to services for clients; Provides service planning and coordinates to identify/arrange/collaborate outcome-focused services and supports that address clients’ needs; Makes recommendations on policy and procedure changes; Maintains case history and progress records, and prepares reports and evaluations in a high volume case documentation environment; Completes permanency plans for individuals under the age of 22 that are in an institutional setting; Assists in identifying and seeking resolution to areas that are barriers to services for clients; Monitors clients’ services and evaluates the effectiveness and adequacy of services through site-based visits and collateral telephone contacts in a variety of settings; Monitors health/safety and reports any abuse, neglect, exploitation or rights concerns; Performs transition planning to ensure continuity of care and management of risk factors; Assist others with solving work-related and other problems that interfere with his/her ability to provide efficient and effective services; Assist in preparing and conducting presentations and orientations; Provides coverage during extended service coordinators leave, vacancies, and assist as needed to provide case load coverage for service coordinators to maintain case load activities; Explains community living options in accordance with program requirements; May perform transition planning to ensure continuity of care and management of risk factors; May train others; May work in one or more program areas, including safety Net Services, Continuity of Services, Texas Home Living, Home and Community-Based Services, Pre-Admission Screening and Resident Review, Community First Choice, and Intake/Eligibility; Assists with Administrative and quality assurance activities necessary to meet unit support needs; Demonstrates and assist with the time management and organizational skills of each service coordinator. Desired Degree/Job Experience: One (1) to two (2) years’ experience in case management or social services work; Graduation from an accredited four-year college or university with major coursework in social, behavioral, human services or a related field is required, as defined in Texas Administrative Code, Title 40, and Rule 2.559 (b) (1); Prefer ability to meet specific requirements of Title 42, CFR, Section 483.430; Prefer Two (2) years’ experience working with individuals with Intellectual and Developmental Disabilities; Prefer bilingual fluency in both English and Spanish.. This position is a Pay Grade 11, with a starting hourly rate of $18.57. This is an non-exempt position. Open until Filled.
